How to apply
- Download the Misc. Consular Service Application Form PDF, fill it in carefully and affix a photograph (2 inches × 2 inches, 5 cm × 5 cm, white background). Only one copy of the Life Certificate will be issued.
- The applicant must visit the Embassy in person. If you cannot come in person, send a notarised letter explaining why, and a medical certificate from a registered medical practitioner, issued no more than 7 days before the request, confirming that you (name) were examined on (date) and are alive on that date. The Embassy may contact the practitioner for clarification. The medical certificate and notarised documents are not required if you come in person.
- If you want the PPO number written in the Life Certificate, bring a copy of the pension book (with your name) or bank pension documents. You may bring the life certificate in the format required by your pension authority in India; otherwise use the sample format PDF.
- Your current valid passport and a self-attested photocopy (the original if you come in person), and proof of valid stay in Chile: copy of visa or valid RUT card.
- Original and photocopy of proof of residence in Chile, such as a utility bill or lease agreement, or an undertaking by a family member if you live with a son, daughter or other relative.
- The Life Certificate for pension is for applicants with Indian passports. If you are entitled to the pension after acquiring foreign nationality, bring the certificate from the pension authority in India stating so.
The Embassy will not email the Life Certificate to the pension authority in India. Sending it is solely the responsibility of the applicant.
